your swing, your stance, or your set-up that causes the problem.

IF YOU HIT IT HERE (TOE)
Improper set-up. Your set up too far away
from the ball. This causes you to hit the ball off the toe, and
the ball heads to the right.
Improper weight shift. Your weight is on the left side during
the backswing, and shifts to the right during the downswing. As
a result, the club comes closer to your body and hits the ball off
the toe.
Early hand release. Your hands released too early on the
downswing, throwing your club into the ground. As a result, you
hit the ball fat, or to the right
IF YOU HIT IT HERE (TOP)
Early arm extension. You extended your right
arm too early coming into the ball. As a result, you hit a fat shot,
hitting the ground behind the ball.
Early hand release. Your hands released too early, with your
right hand doing all the work. You'll hit a fat shot, or a shot
to the right.
Over the top. You are coming over the top
during your downswing (i.e, not staying behind the ball at impact).
This causes the club face to open (ball heads to the right), or
close (ball heads to the left)
IF YOU HIT IT HERE (HEEL)
Early weight shift. You are in front of the
ball at impact. When you shifted your upper body first, you got
ahead of the ball and hit it on the inside. This causes you to shank,
or hit to the right.
Late hand release. Your club face has not had a chance to
square up, causing you to strike the ball with the heel of your
club.
Outside-In-Swing. On your downswing, your right arm releases
too soon, pushing the club away from your body, and you come accross
the ball, not through it. You strike the ball towards the heel,
which can result in a shank.

IF YOU HIT IT HERE (BOTTOM)
Steep angle. Your angle was too deep, and
you club came down on the ball too much. This pushes the ball well
right, or hooks it left.
Over the top. You didn't stay behind the ball at impact,
and your club face opened, causing the ball to sail right.
Moving head. You moved your head, which caused you to pull
away at impact, hitting the top of the ball with the bottom of your
club
IF YOU HIT IT HERE (HEEL)
Increased tempo. You were swinging too fast. As a result,
your club moved away from your body and swung out around the ball.
This causes you to pull or slice the ball.
Standing too close/Placing the ball too far back in your stancce/Swaying
at impact. These will cause you to hit the ball off the heel.
IF YOU HIT IT HERE (TOE)
Pulling away. You came out of the shot early
and pulled away. The result is a weak shot.
Reverse pivot. You shifted your weight from front to back
on downswing, or you picked your head up on downswing. This caused
you to hit the ball off the toe of your club.

IF YOU HIT IT HERE (TOE)
Improper set-up. You set up too far away from
the ball, causing you to hit off the toe of the club and to send
the ball out to the right.
IF YOU HIT IT HERE (HEEL)
Improper set-up. You set
up too close to the ball. This will move your club out and pull
the ball to the left.
IF YOU HIT IT HERE (BOTTOM)
Too much wrist. You are stroking with too
much wrist and trying to help the ball to the hole. This causes
your blade to come up too quickly and to hit the ball on the bottom
edge of your putter.
